Chasing clients for documents
Your case or practice system, a shared mailbox and the folder or client portal where documents land.
A clinic gets new patient forms and consent forms back before the appointment by sending both when the booking is made, chasing them on two fixed days and giving the front desk a list the day before of who has not returned them. Treat them as two jobs. The questionnaire can be finished at home. The consent form can be sent and read ahead, but consent itself is a conversation between the patient and the clinician, and the signed form is only its record. An AI worker can send both forms, chase them and produce the day-before list. It cannot take consent.
What does a new patient need to return?
A new patient at a physiotherapy, optometry or aesthetics clinic needs a medical questionnaire and a consent form before the clinician can begin, and a 30-minute first session that opens with ten minutes of form filling is a 20-minute session.
Where do the forms go missing today?
The forms go out. The booking system sends them with the confirmation, where it has been set up to.
The patient completes them, or does not. Nobody is watching which.
The gap is found at the desk. The receptionist learns on the day who has not done theirs.
The clinician reads the questionnaire. They read it before or at the start of the session, and what it means for treatment is theirs to judge.
The clinician takes consent. They explain what is proposed, and the patient agrees or does not. The standard comes from the clinician's own regulator, where there is one. For a physiotherapist the HCPC's standards require valid consent, voluntary and informed, before any care or treatment is given.
The form is filed. The signed form goes on the patient's record.
How to get both forms back without rushing consent
Separate the two forms. The questionnaire collects facts and can be finished at home. The consent form records a decision made with the clinician, so sending it ahead lets the patient read it and bring questions, and does no more.
Tell the patient what the form buys them. Say how long the questionnaire takes, and that doing it at home leaves the whole first session for the clinician.
Pick two chase days and keep to them. Use three days before and the afternoon before, and name the missing form in each message.
Give the desk a day-before list. It shows which of tomorrow's new patients to ask in early, and which clinician to warn.
What can an AI worker do with the forms?
Here an AI worker can take step one, chase the patient through step two and replace step three with a list the day before. The worker is given a job description, a named supervisor at the clinic and a review every month. It sends the questionnaire and the consent form when the appointment is booked, in wording you have approved, and reminds the patient on the days you choose. It works from the status of each form, returned or not returned, and is not shown the answers. It resends a link that will not open, answers practical questions from what you publish, such as where to park and what to bring, and gives the front desk the day-before list. Each month it reports how many new patients arrived with both forms returned. The role is specified under chasing patients for documents. Insurer invoices, enquiries and the other front-desk roles are on our page for private clinics.
Consent, proof of age and what else stays with a person
Consent. Only the clinician can explain the treatment and satisfy themselves that the patient agrees freely and understands. The agent chases the form so that the patient has read it. Where your clinicians want it signed only after the conversation, it asks the patient to read it and bring it. It never tells a patient what a treatment involves, what its risks are or what result to expect.
Reading the questionnaire. The clinician reads every one. The agent has no opinion on what a patient has written.
A question about symptoms or suitability. If a patient replies asking whether the treatment is right for them, or describing how they feel, the message goes to a clinician. The agent does not judge whether it is urgent.
Proof of age in an aesthetics clinic. In England it is an offence to inject botulinum toxin or a filler for a cosmetic purpose into a person under 18, or to book it for them; the one exception needs a doctor's approval. The government's guidance is that proof of age is asked for before an appointment is made and again before the procedure, and that looking over 18 is not enough. Checking the document is a person's job, and so is any patient who gives an age under 18.
Patients who need help with a form. Someone who cannot use an online form, or who needs it in another format, is helped by the front desk, by phone or in person.
What the agent may open. A questionnaire holds data concerning health, which the ICO lists as special category data under UK GDPR, so it needs a lawful basis and a separate condition for processing. The clinic settles both, and the role's written job names the parts of the booking system the agent can open.
What to settle before the first form goes out
The forms each kind of first appointment needs.
Whether your booking system can report a form's status without the answers.
A count of last month's first sessions that started late for a form.
The message wording, the chase days and who receives the day-before list.
The clinician who takes any message not about the form itself.
Where forms sit among the work that grows with every appointment is in our article on scaling a private clinic without hiring more staff.
Your clinicians would keep the reading and the consent conversation, and an AI worker would get the questionnaire and the consent form back before the day. Count last month's first sessions that began at the clipboard, then talk to our agent.
