Send the medical history form when the appointment is booked, chase it on fixed days, and give reception a list each afternoon of tomorrow's patients whose form has still not come back. Practice software can already send the link. Without the chase and the list, the gap is found when the patient is standing at the desk. An AI worker can run both, and need not read a single answer. The dentist still reviews every history with the patient.

Why does every treatment appointment need a form?

The GDC's standards require complete and accurate patient records, including an up-to-date medical history, each time a patient is treated. So every appointment for treatment needs either a new form or a confirmation that nothing has changed, and every extra chair adds to the number going out each week.

Where does the form go missing between booking and the chair?

  1. The form goes out. The practice software or its patient portal sends a link, usually with the booking confirmation or the reminder.

  2. The patient completes it, or does not. Nobody looks until the day.

  3. The gap is found. A receptionist sees the form is missing when the patient checks in.

  4. The form is done at the desk. The patient fills it in on a tablet or a clipboard in the waiting room, while the dentist waits and the appointment runs late.

  5. The dentist reviews it. They go through it in the surgery, with the patient, before treatment, and anything that has changed is theirs to weigh.

Steps one and five work. Between them, for days, the practice does not know who has done the form, and by the time it finds out there is no time left to do anything about it.

How to get the form back before the day

  • Send it at booking, not with the reminder. A patient who gets the link a fortnight ahead has a fortnight to do it.

  • Say why and how long. "Your dentist needs this before they can treat you. It takes about three minutes" is answered more often than a bare link.

  • Chase on fixed days. Pick them, for example a week before and two days before, and name only what is missing.

  • Run the list the afternoon before. Tomorrow's patients whose form has not come back get one last message asking them to complete it tonight or come ten minutes early.

  • Keep health details out of email. Tell patients to use the form, not to reply with their medicines, because the form goes to the record and an email sits in an inbox.

Which part can an AI worker take on without reading the form?

The stretch between step one and step five can go to an AI worker. Its duties are written down, someone at the practice is named to check what it does, and it is reviewed every month. It can send the form link when the appointment is booked, in wording you have approved, and remind the patient on the days you choose. It works from one fact about each form, returned or not returned, where your software can report that without showing the answers. It can resend a link that will not open and answer practical questions about the appointment from what you publish. Each afternoon it gives reception the list for the next day, and each month it reports how many forms were back before the day. The role is set out under chasing patients for documents. Recalls, private treatment enquiries and the rest of the desk's written work are on our page for dental practices.

What stays with the dentist and reception?

  • Reading the history. The dentist reviews every medical history and confirms it with the patient. The agent forms no view of a patient's health and is not shown the answers.

  • A reply with health details in it. If a patient emails about a condition, a medicine or a symptom, the agent does not answer the content. It passes the message to your team and asks the patient to put the details on the form.

  • Patients who cannot use an online form. Reception helps them on paper or a tablet, as now, and the list tells reception who they are before the day. Anyone who needs a call gets it from reception.

  • The data protection decisions. What a patient writes on the form is data concerning health, which the ICO lists as special category data under UK GDPR, so it needs a lawful basis and a separate condition for processing. Those are the practice's decisions. The GDC also expects patient information to be kept confidential, secure and used only for the purpose it was given. What an agent may see is agreed in writing before it starts.

  • Whether to treat without a form. That is the dentist's call on the day, never the agent's.

What to settle before you begin

  • Your own rule on which appointments need a full new form and which need an update.

  • How your software reports a form's status, and whether it can do so without the answers.

  • The message wording and the chase days.

  • A count of the forms completed at the desk in one ordinary week, which is the number to bring down.

  • Who receives the afternoon list, and who is told when a patient replies with something clinical.

Forms are one of the lists that grow with every chair; the others are in our article on scaling a dental practice without hiring more staff.

Your dentists would still read every history; an AI worker would send and chase the forms and hand reception tomorrow's list. Do surgeries start late because a form is not back? Talk to our agent about the afternoon list.
